We want your knowledge and expertise!Job Summary and QualificationsCASE MANAGEMENT SOCIAL WORKER MSW - PRN DAY SHIFT [Must be available for a minimum of 6 8-hour shifts bi-weekly]This position will support the East POD of Medical City Healthcare and can work at any of the following facilities: - Medical City Dallas, Medical City Denton, Medical City Frisco, Medical City Heart & Spine, Medical City Lewisville, Medical City McKinney and Medical City Plano*PREMIUM FLOAT POOL - PAY RATE The Case Management Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
  • Acts as a liaison between the patient and community resources/post-acute care providers
  • Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care
  • Assists the Case Manager RN in facilitating patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput What qualifications you will need:
    • Master of Social Work (MSW) required
    • Licensed Masters Social Worker (LMSW) or License Clinical Social Worker (LCSW) required
    • Certification in social work or case management preferred
    • Acute care social work experience preferred - This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).Medical City Healthcare strives to deliver excellence.
