Job Description:
A company is looking for a Community Health Physician to provide
general supervision for Community Health Workers in local
communities.
Key Responsibilities
Conduct real-time telehealth visits with patients referred by
Community Health Workers
Review Community Health Worker documentation and provide brief
monthly notes and billing for services rendered
Respond to urgent clinical issues and assist patients in finding a
primary care provider if needed
Required Qualifications
MD or DO degree with a valid medical license in Indiana
Experience in primary care or telehealth services
Ability to work 5-10 hours per week
Familiarity with electronic health record (EHR) systems
Commitment to addressing health inequities in the community
