Access Center Nurse coordinates, facilitates, and communicates as a
representative of the Access Center to promote optimal patient care
and support the mission of Children's Health. Ensures efficient
patient flow, system level bed management, communication, and
excellent customer service to referral hospitals that are
contacting Children's Health for patient transfers and transport
team requests. The Access Center Nurses serve as liaisons and
resources for all Access Center staff, community hospitals, and
clinical units/services across the healthcare
enterprise.Responsibilities
- Provides oversight for all clinical escalations and notifies
the Access Center Medical Director as appropriate.
- Provides oversight for system level bed management to all
Access Center staff and triages patients' needs for ICU and complex
admissions.
- Facilities daily shift huddles for bed management, which
includes physician huddle and Access Center shift huddles.
- Escalates any capacity constraints or concerns to Access Center
leadership and Medical Director.
- Applies clinical expertise to triage and prioritizes referrals
while working in collaboration with Access Center staff.
- Follows appropriate policies/guidelines for patient referrals
and transport team requests.
- Anticipates care issues and promotes optimal outcomes by
utilizing appropriate delegation of patient care.
- Adheres to all regulatory requirements for the patient transfer
process and bed management.
- Assists with coordination for the correct team configuration,
the right mode of transport, and ensures the right equipment is
available based on patient resource needs and acuity.
- Completes a rapid assessment of patient information to ensure
the correct consulting physician is contacted and escalates for
Medical Director evaluation when indicated.
- Authors and distributes Q12 hour report.
- Tracks and resolves referral delays.
- Monitors a continuous count of bed capacity across the system
and assists communication specialists during bed capacity alerts in
regards to referral and Access Center processes and ensures
optimization of bed resources across the system.
- Assists all transport staff in problem solving and
in-the-moment conflict resolution.
- Promotes education and positive learning and educational
materials for staff.
- Audits intake calls and assists with call reviews to identify
improvement opportunities.
- Assists with review of Safelink reports related to the Access
Center.
- Identifies and resolves barriers and special issues for complex
transports and coordination of care for the transport team.
- Facilitates team huddles at beginning of shift.
- Utilizes crisis incident stress management experience and
provides debriefings with staff when requested.
- Provides efficient and effective hand-off between shifts to
ensure communication of vital information required to maintain safe
operations and continuity of services.Work ExperienceHow You'll Be
Successful:
- At least 2 years Nursing experience in Critical Care or the
Emergency Department Required
- At least 2 years Nursing experience in Transport or EMS
PreferredEDUCATION
- Four-year bachelor's degree or equivalent experience Bachelor
of Science in Nursing Required
- FEMA ICS 100, 200 RequiredLICENSES AND CERTIFICATIONS
- Current license to practice professional nursing in the State
of Texas Upon Hire Required
- Basic Life Support for Healthcare Providers as required by CP
1.20 Life Support Course Requirements Upon Hire Required
- Pediatric Advanced Life Support (PALS) certification Upon Hire
Required
- Certified Flight Communicator 1 Year Required
- Licensure/certification as a paramedic or EMT PreferredA Place
